1: What is TSH, T4, and T3?
Thyroid-stimulating hormone/thyrotropin/TSH is a pituitary hormone that stimulates the thyroid gland to produce thyroxine (T4).
After the release of T4 triiodothyronine (T3) is released which triggers the metabolism of almost every tissue in the body.
To know the patient is having hyperthyroidism or hypothyroidism, TSH concentration is measured.
If the person having excess amount of TSH concentration, it means that person is having hyperthyroidism.
Similarly, if the person having less amount of TSH concentration then we’ll say that the person is having hypothyroidism.
The result analysis depends on both the TSH and T4 concentrations. Also, the T3 measurement is also useful.
2: If The TSH/T3/T4 level Is Higher:
Thyroid-stimulating hormone(TSH): High TSH levels are a sign of an underactive thyroid (hypothyroidism).
The pituitary gland produces more TSH in order to stimulate the thyroid gland to produce thyroid hormones.
Free triiodothyronine (FT3) and free thyroxine (FT4): High levels of free thyroid hormones in the blood may be a sign of an overactive thyroid
3: If TSH/T4/T3 Level Is Lower:
Thyroid-stimulating hormone(TSH): Very low TSH levels in the blood may be a sign of an overactive thyroid (hyperthyroidism).
The pituitary gland then produces less TSH, in order to stop “telling” the thyroid gland to make more hormones.
Free triiodothyronine (FT3) and free thyroxine (FT4): low levels could be a sign of an underactive.
4: TSH, T4, T3 Normal Range:

TSH – between 0.4-3.8 microIU/ml (micro-International Units per mili-liter),
T4 – 4.4-12.5 micrograms%
T3 – 0.9-1.95 ng/ml (nanograms per milliliter)

5: If TSH T4 T3 Normal Range Not Observed:
These diseases might be the cause of the disbalance of the normal range.
5.1: TSH, T4, T3 Level Increased then:
These are the diseases that increase the production and disbalance the normal range.
- Hashimoto’s thyroiditis (an auto-immune disorder where the immune system of own body attacks the thyroid gland and decreases the production of hormones that are required for the body)
- Grave’s disease (an immune system disorder which results in the overproduction of thyroid hormones)
- Subacute thyroiditis (acute inflammation of the thyroid gland)
- Toxic adenoma (increased hormone production from single large nodule/lump in the thyroid gland)
- Toxic multinodular goiter, Thyroid cancer
5.2: TSH T4 T3 Level Decreased then:
These are the causes that increase the production and disbalance the normal range.
- Iodine deficiency or excessive iodine
- Pituitary dysfunction
- Idiopathic myxedema (severe hypothyroidism)
- Starvation, or long-term illness
If TSH levels alone increased and T4, T3 levels are within the range. It may be due to mild hypothyroidism.
6: If You Observe The TSH T4 T3 Normal Range:
If you observe the normal range it means the thyroid gland is functioning normally and no need for further medical intervention.
